Leslie Ann Boyle-Gagnier, age 64 of New Smyrna Beach, Florida, passed away at Halifax Hospice Care Center in Port Orange, Florida on July 8, 2018.
Memorial Service will take place on Sunday, July 15, 2018, 11:00 AM at Baldwin Brothers Funeral Home, 620 Dunlawton Avenue, Port Orange, FL 32127.
In lieu of flowers donations can be made to Halifax Health Hospice: 3800 Woodbriar Trail, Port Orange, Florida 32129.
